After notifying an appointed Livestock Valuer within 48 hours of the discovery of a kill or injury to livestock or poltury, complete the following form to inform the Clerks' department of an expected claim through the Ontario Wildlife Damage Compensation Program (OWDCP).

Please Note: Completing this form is not the notification to the livestock valuer, you must contact the livestock valuer seperately.
This form is to notify the Township that you have contacted a valuer to attend your property.
